BILLING FAQ's

We understand that you may have some questions following your experience with us. For your convenience, we have compiled a list of our most Frequently Asked Questions and provided answers and explanations that we hope will satisfy your inquires. Thank you!
Why didn't my insurance cover the bill?
Your insurance company did not consider your transport medically necessary. We recommend you contact your insurance company to explain the situation and ask that your bill be re-processed.
Why don't you participate with most insurance companies?
Diaz Memorial Ambulance Service does participate with Medicare and Medicaid government insurances.

Diaz Memorial Ambulance Service has attempted numerous times to negotiate a reasonable contract with private insurance companies. Unfortunately, private insurance companies will not reimburse Diaz Memorial Ambulance Service at a rate that covers our cost to provide the service.
What is a deductible? Co-Pay?
A deductible is an amount you must spend and pay for out of pocket before your insurance policy will begin covering medical bills. 

A Co-pay is an amount you must pay for each office visit or ambulance call, toward the total cost of the service charge. An ambulance Co-pay is usually $50 – $300 per call, depending on your insurance carrier. For information regarding your co-pay, please contact your insurance company directly.

Co – Insurance is a percentage of charges the patient must pay after the deductible is met. Most Co-insurance range from 10% or 20% of the charges. ​For information regarding your co-insurance, please contact your insurance company directly.
Was my bill/claim submitted to my insurance?
If we have your most current information, your bill was submitted. If Diaz Memorial Ambulance Service has your health insurance information, we will submit a claim to your Insurance company for our services. If we do not have your insurance information, we will mail you as the patient an invoice for our services.
What is the charge for ambulance service?
ALS 2 Emergency - Ambulance Service Advanced Life Support, Emergency Transportation
Level Two (2) - $1275.00

ALS 1 Emergency - Ambulance Service Advanced Life Support, Emergency Transportation
Level One (1) - $1010.00

BLS Emergency - Ambulance Service, Basic Life Support, Emergency Transportation - $800.00

Paramedic Intercept - Paramedic on board another ambulance, Assisting with patient care and/or instituting Advanced Life Support Services - $950.00

Ground Mileage - ALS/BLS Ground Mileage, per loaded mile - $17.00

All service charges include all medications, IV supplies, cardiac interventions, trauma supplies & equipment used on every transport. Also, it is important to know we cannot itemize your bill for these items.
